Well, lets address the first question -
1. Lou Alessi does it this way, and of course, I often (read almost always

) look to Lou for guidance. The felt is firm, so when you tighten it down, that is where it will stay. I have not tried sheep skin, but it seems to me like it would let the holster move around too much. The felt is 100% wool, and wool absorbs LOTS of water. This means that as you sweat, the felt absorbs the moisture and does not let it stay against your skin. This keeps you from feeling sweaty/damp under the ankle rig, and then your body heat causes the moisture to evaporate. I don't know if this would happen with sheep skin, since it usually has a bit of lanolin in it, and that might prevent it from absorbing moisture. Of course that is just speculation.
I have one of Lou's rigs for my J Frame, and I cary it every day and forget it's there. Figured that was a good reason to stick with the proven recipe.
And for the second....
2. Yes, for those that want them, thumb breaks will be available.
